Practical Tools for Dementia Diagnosis, Intervention and Care:
Ready-to-use checklists, policies, handouts and training on CD-ROM
On 3 February 2009 the government launched Living Well with Dementia: A National Dementia Strategy to
transform the quality of dementia care. This affects a range of health and care professionals, including:
- GPs
- Care home managers
- Domiciliary care managers
- Community psychiatric nurses
- District nurses
- Social workers
- Hospital and hospice staff.
In order to help health and social care professionals to implement the National Dementia Strategy (NDS), we
have developed Practical Tools for Dementia Diagnosis, Intervention and Care.
With this practical CD-ROM and accompanying handbook, you will be able to:
- Improve professional awareness and understanding of dementia and provide an informed
and effective workforce (NDS objectives 1 and 13) using our PowerPoint presentations covering
key issues such as communicating with people with dementia, and recognising and understanding
symptoms.
- Provide good quality early diagnosis and intervention for people with dementia (objective 2)
using our straightforward care planning tools, which include needs assessment checklists for:
assessing cognition skills; assessing behaviour; evaluating activity limitation; and assessing quality
of life.
- Provide helpful and accurate information for those with diagnosed dementia and their carers
(objective 3) and improve end of life care for people with dementia (objective 12) using our
handouts on key issues including: short breaks for patients and their carers; explanations of
different types of dementia; making choices about dementia care; and recognising the symptoms
of dementia.
- Assure the best services for people with dementia by implementing an effective quality
assurance system with our checklist for auditing provision of dementia services and policies on:
handling challenging behaviour; emergency evacuation of vulnerable people; physical intervention
and restraint.
This publication also covers additional requirements for care homes:
- Improve the quality of care for people with dementia in care homes and prepare for inspection
(objectives 11 and 15) with our checklists for auditing provision of dementia services and model
policies: handling challenging behaviour; emergency evacuation of vulnerable people; physical
intervention and restraint; and outings risk assessment.
Updates are issued quarterly to reflect modifications to legal aspects or content, as well as for functional
