Tools and Support for Advance Care Planning and Palliative Care
Practical checklists, policies, information sheets and training on CD-ROM

In July 2008 the Government launched its End of Life Care strategy as a benchmark to help improve the quality of care of people at the end of life. This affects a range of health and care professionals:

  • Palliative care professionals must assess the needs of terminally ill patients and provide treatment and care, as well as supporting relatives, carers and friends, and implementing the strategy
  • Care home managers are required to have policies and procedures in place for ageing and death to meet the National Minimum Standards relating to palliative care; also to demonstrate effective provision of palliative care in their AQAA and supporting evidence for inspection
  • Doctors will soon be annually assessed and required to revalidate their licence under GMC standards based on the Good Medical Practice framework, which includes issues related to palliative care and death.

With this practical CD-ROM and accompanying handbook, you will be able to:

  • Understand the law regarding end of life care with our straightforward guidance on key issues such as the Mental Capacity Act 2005, lasting power of attorney, 'do not attempt to resuscitate' orders, living wills and statements of wishes and preferences.
  • Protect patients' dignity and privacy and ensure patients and their family are treated with sensitivity and respect, with best practice template policies on equality in end of life care and palliative care.
  • Involve and support relatives, carers and partners in decisions about terminal care and death with information sheets on key issues such as: making choices about palliative care; complementary approaches to palliative care; adapting to death, dying and bereavement.
  • Reduce the number of emergency hospital admissions with our needs assessment checklist, advance care planning record and statement of wishes and preferences.
  • Save money on training staff with our PowerPoint presentations covering key issues including: advance care planning; recognising the end of life phase; and communication skills.

Care homes 18-65

In addition, care home professionals in care homes for adults aged 18-65 will be able to:

  • Meet Standard 6 Service User Plan and meet the needs of residents suffering from a terminal or long term illness using our assessment checklists for care planning review; religious or spiritual needs assessment; physical, learning or mental disabilities assessment.
  • Meet Standard 21 Dying and Death and ensure residents and their families are treated with sensitivity and respect using our best practice policies on: resuscitation; equality in end of life care; and palliative care.

Care homes 65+

Care home professionals working in care homes for older people aged 65+ can use the publication to:

  • Meet Standard 7 Service User Plan using our assessment checklists for: care planning review; physical, learning or mental disabilities assessment; religious or spiritual needs assessment.
  • Meet Standard 10 Dignity and Privacy and ensure residents are treated with respect with our worked example policy on dignity and privacy in end of life care.
  • Meet Standard 11 Dying and Death and ensure residents and their families are treated with sensitivity and respect with our worked example policies on: resuscitation; equality in end of life care; palliative care; and our information sheet on making choices about palliative care.

Health professionals

Health professionals can also use the publication to:

  • Put into effect systems to protect terminally ill patients and improve care using our checklist auditing palliative care provision, patient feedback form and complaints policy.
  • Access practical guidance on working with community nurses, charities, hospitals and other services to provide consistent treatment and support.
